Starting in January 2020,
Eye Vision Associates are happy to announce our partnership with the Smithtown Guide Dog Foundation & America’s VetDogs.

Eye Vision Associates will be delivering all donated items, monthly, to the Smithtown Guide Dog Foundation to help them in their effort to help provide seeing-eye dogs to the visually impaired.

If you would like to participate please feel free to drop off any needed items in our donation boxes!

ITEMS NEEDED

  • Kong Extreme Rubber Toys – Black (L, XL, XXL) – New Only
  • Nylabone (new only):
  • DuraChew
  • Big Chews
  • Galileo Chew Toys: Single-molded bones
  • “Wolf” or “Souper” sized
  • “All” brand liquid laundry detergent only (new only)
  • Clear shipping tape in dispensers & refills
  • Yellow “caution” tape in rolls
  • “Command” Utility Hooks – medium sized (new only)
  • Construction Cones & Barrels – Large; for obstacle work
  • Dish Sponges with Scrubber Side (new only)
  • Gently Used clean large Bath Towels – any color
  • Gently Used clean fabric/new material; min size (2’ x 3’)
  • Gently used clean baby blankets or fleece blankets
  • Gift Cards: Panera, Starbucks, or Target for students on class
  • Heating Pads (New); unused in original packaging for students to use on class
  • UL Approved Interior 9 foot extension cords (any color; new)
  • Clean / Empty 17lb plastic kitty litter pails with covers
  • Lint rollers “Scotch Brite” brand (new only)
  • Lysol Disinfectant Spray
  • Lysol Toilet Cleaner
  • Lysol Hand Soap
  • New Large White Bath Towels for student use on class
  • Paper Towel Rolls (new only)
  • Poop pick up bags (new only)
  • Pens (blue or black), “Wite-Out” correction tape

Mask Policy

In light of New York’s lifting the mask mandate, as of February 12, 2023, masks are no longer considered mandatory at our office.

We still request that any patients or staff who have been around anyone with COVID, RSP, or the Flu, to please, out of respect for all of our patients and staff, continue to wear a mask while in our public space.

We appreciate everyone’s kindness and understanding.
